Everyone blaming Moreno(myself) and Lovren/Mingolet a bit. Dixon is saying Gerrard was mostly at fault for not following the runner. On retrospect, looking at the whole first half, Gerrard's positioning and covering of runners from midfield have been terrible. On numerous occasions our back four were so separated from our midfield, just a big empty space where our DM should be.

They definitely aren't blameless. In Mignolet's case he shouldn't have been put in that position in the first place. But yes, one would expect him to do a bit better. That was a bit Reina-esque.
