It was a split second after a massive hoof and eyes follow the ball. Mine did while watching the game. Noticed it in the peripheral but not enough to see the impact clearly enough to know what happened. Feasible that the ref saw the same, and VAR could only intervene if it judged it to be a possible red card, for some reason. This is one of the (many) issues with VAR, as you say I can see why the ref might have missed it but the purpose of VAR is supposedly to catch incidents that the ref might have missed and stuff like this slips through all the time.
