The decision to take Mascherano off - Rafa should have take Alonso off, because Masch had Kaka in his pocket. When he left, Kaka was free and it was all over.

Maybe Crouch should be introduced earlier too, but I think when Mascherano was substituted, we lost the final right there and then.
