Pushing with a closed fist? I was watching it in HD earlier on a big screen (I think ESPN had a different angle too) and I thought it looked like a punch. A quick search and it wasn't too hard to find replays online.

Someone else can confirm as I was only half watching as I knew the result, but I think the commentator mentioned they had a few scuffles (or some sort of confrontation) already during the match.

Surely they effectively have one every couple of years when they all play each other in world cup qualifying?
