I have my own theory on that and its far more simple. He has the whole squad in the changing room at half time and talks about the second half. When they go back out the player coming out then has a good 5 minutes to warm up and once that is done the change is made. Other than that they would come on to the pitch very cold, and therefore a greater chance of injury
