This isn't the first time Nadal throws away a set when a break down feigning injury and recovering when the next set starts. I'll dig it up for you but there is certainly some justification into thinking this. He has a history of taking suspect medical time out when losing.

It's a final of a grand slam, nobody throws a set away in one of these especially then being 2-0 down.

He's relaxed now. Helps when you have muscle injuries. It's clear he's not well.
