£45 might be a lot of money to some of the lads playing but it's a lot less than some might lose through having to take a day/week off work after an 'enthusiastic' challenge from the type of muppet that thinks of himself as a 'character'.
I don't think that there is a suggestion that it is being used as a cash cow, my experience with this type of football (admittedly nearly 10 years ago) was that the referees were largely reluctant to give out yellow and red cards, even for fairly bad challenges. You got a few referees who were complete tossers though, we had a 5 aside game where a load of us got booked for questioning refereeing decisions. - One included a goal that was disallowed because our player supposedly kicked it out of the GK's hands, for which the referee awarded a freekick outside the area, when asked why the freekick was there he said that was where the offence took place, when asked if that was the case surely the GK handled the ball outside the area he said no and started booking our players

I don't know how wide spread bookings are in the kids game, I know that they will want to discourage bad behaviour in players at a young age but £12 for a booking can be alot for kids/parents in poor areas, that doesn't seem right.

He'll be promoted to the Premier League in double quick time

Yeah, definitely a problem in kids football. The bad behaviour at a very young is probably linked to simply that they are kids and harder for them to control their emotions so would be good to reduce the fine to a token amount or even replace it with something else. Got no issue with it staying for lads in their 20s upwards who are just being tossers though.

No I think that there is a different issue with the adults and kids though, the young kids will almost certainly have their fine paid by their parents, maybe they don't get any pocket money or something to pay for it, but I would have thought that a ban system would be more effective for the kids, if they aren't able to play the game because of their bad behaviour that's probably a more effective punishment that they are likely to learn from.

The adults it's a different situation you get a whole range of people playing, for some it's probably the highlight of their week, to others it's a chance to kick lumps out of people with little or no come back, it very much depends on the level.

I just couldn't be arsed filling out paperwork, and I rarely remembered the offence anyway! I refereed when I was 18, got some beer money and had a laugh at how ****e the football was

That was the impression that I got from most of the referees that did our games, it's a few quid for doing very little (most of our refs didn't even run, just stood by the half way line)
